Tree Removal
For Tree(s) removal, attach a picture(s) and description in the Architectural Change Request Description field. Tree(s) removal do require plans / plot drawing with replacements tree(s) location(s) as well. If the tree(s) are not clearly dead or diseased the Creekside BOD will then require a professional arborist sign off that tree removal is necessary.
Adjecnet Homeowners Approval
THE BOARD MAY REQUIRE ADJACENT HOMEOWNERS TO APPROVE OUTSIDE CHANGES. Surrounding neighbors are often impacted by the Architectural Control Committee (A.C.C.) request changes, yet are not involved in the review process. As such, the A.C.C. may not understand the potential negative impact your request might have. In order for this request to proceed, this form requires you to obtain 3 approvals from surrounding neighbors that could be affected. Provide 3 home addresses, homeowner names and emails.

City Permits:
If a city permit is applicable, THIS SIGNED (APPROVED) FORM MUST ACCOMPANY ANY PERMIT REQUEST TO THE CITY OR IT WILL BE REJECTED. CITY PERMITS ARE NOW SUBJECT TO BOARD APPROVAL! All supporting documents WILL NOT be returned, as they will become a part of the permanent record in your homeowner file. A decision will be mailed to you within 30 days from date of receipt of the application.
Local Building Regulations and Restrictions:
UNDER NO CIRCUMSTANCES DOES APPROVAL BY THE HOMEOWNER’S ASSOCIATION INDICATE FULL AUTHORIZATION. In addition, it maybe necessary to obtain building permits and structural approval as required by the City of Rolling Meadows. All improvements are subject to local building regulations and restrictions of record. If the homeowner proceeds without all necessary approvals, he/she maybe subject to legal proceedings as well as having to remove all unauthorized improvements at his / her expense. Approval of similar plans and specifications shall not be construed to guarantee another Homeowner’s planned improvement.
Request Acknowledgement
I acknowledge that the information submitted is correct and I have read and understood the Creekside at Plum Grove 'Rules Regarding Use and Appearance of Lots' pertaining to my request. I also acknowledge that any deviations from approved plans require re-submittal to the Creekside Board prior to proceeding.

By providing your initials, you confirm your intention of executing the terms in this Creekside at Plum Grove 'Architectural Change Request Form' and the Creekside at Plum Grove 'Rules Regarding Use and Appearance of Lots'. By providing your initials you acknowledge that if work commences PRIOR to a decision and without a written response signed by the current Creekside President board member, a minimum non-negotiable $500 fine will be imposed plus it can be required to remove any and all work at my own expense.
The Creekside Board of Directors will notify the applicant within thirty days of receiving this form. The Applicant will either receive notification of a final resolution or be requested to provide further information. The applicant is strongly advised not to proceed with any rule exception prior to receiving Board approval as he/she will be subject to monetary fines. THIS CHANGE TO BE COMPLETED WITHIN 60 DAYS FROM COMMENCEMENT OF CONSTRUCTION. APPROVAL VALID FOR SIX MONTHS. Homeowner is liable for any charges incurred as a result of contractor negligence.
